1. Introduction

This document describes projected household expenditure data from the 2002 Medical Expenditure Panel Survey (MEPS) Household Component file aligned with the 2002 National Health Expenditure Accounts (NHEA). MEPS is issued by the Center for Financing, Access and Cost Trends (CFACT), within the Agency for Healthcare Research and Quality (AHRQ) and the NHEA is issued by the Office of the Actuary within the Centers for Medicare and Medicaid Services (CMS). The 2002 NHEA-aligned MEPS is a product of the Division of Modeling and Simulation (DMS) within CFACT, produced with the cooperation of CMS1.

Projected expenditures are calculated in a two step process. First, core data from the 2002 NHEA-aligned MEPS file are projected to each end year through 2016 by adjusting MEPS person weights using Census projections2 for population totals, fertility and mortality over time. Then, the re-weighted NHEA-aligned MEPS expenditures are calibrated annually by type of service and source of payment categories so that growth in the re-weighted NHEA-aligned MEPS expenditures matches growth in the projected NHEA 3,4. Users should be aware that these estimates are surrounded by uncertainty from a wide variety of sources detailed in Section 5 of this document5.

The projection process yields a set of annual data files and summary statistics available for public use for all each year, 2003 through 2016. These files, named PMEPyy.DAT6, contain person level data with (i) individual level medical expenditures classified by type of service and source of payment categories, (ii) a person weight that enables national-level estimates, (iii) individual level demographic characteristics used in the aging procedure and iv) insurance and poverty status variables, not controlled for in the aging process7.

Each of the aged PMEPyy.DAT files contains a sample of 37,418 respondents. The files contain one record for each person and are designed to provide estimates that are representative of the civilian, non-institutionalized population of the US in the designated end year. While each record is distinguished by a person-level identification number, this variable does not enable researchers to link data from the projected NHEA-aligned MEPS files to any other MEPS data releases8, including the 2002 NHEA-aligned MEPS.

Tables are available to provide summary statistics on various characteristics from each year of the projected NHEA-aligned MEPS files as well as from the 2002 NHEA-aligned MEPS file.

2. Medical Expenditures: Type of Service and Source of Payment Categories

Each person record contains data on the expenditures made within seven different types of service categories plus a total expenditure category. Total expenditures are calculated by adding expenditures for all seven categories9. The categories and their two to three character abbreviations are as follows:

2. Physician: PHY
3. Other Medical: OTH
4. Home Health: HHC
5. Other Provider: OBO
6. Dental: DVT
7. RX: RX
8. TOTAL: TOT

Previous users of MEPS Projected Expenditures will notice that the current data release no longer contains expenditure details at the sub-category level11. This level of detail is not available on the NHEA-aligned MEPS file, and therefore cannot be projected to future years.

The expenditures in each category are divided into nine sources of payment, plus a total. Total expenditures (EXP) equal the sum of the nine source of payment expenditure variables12. Those categories and their two to three character abbreviations are as follows:

1. Out of Pocket: SLF
2. Private Health Insurance: PHI
3. Medicare: MCR
4. Medicaid: MCD
5. Tricare: TRI
6. VA: VA
7. Workers Comp: WC
8. Other Public: OTP
9. Other: OSR
10. TOTAL: EXP10

Expenditure variables are named with the following convention: TosSopYY (i) the first two to three characters of the variable name contains the type of the service code, (ii) the next two to three characters contains the source of payment code and, (ii) the final two numbers are the last two digits of the end year. For example, if an individual had an out of pocket dental expense and a researcher wanted to access the projected expense in 2015, the variable of interest would be DVTSLF15. (i) DVT is the three character type of service code for dental expenses, (ii) SLF is the three character source of payment code for out of pocket payment and, (iii) 15 represents the end year 2015. There are a total of 80 expenditure variables for all the type of service variables crossed with all the source of payment variables above. A full array of the 80 TOS by SOP variables can be found in Table 1 in the appendix.

3. Population Aging

Each person record includes a person-level sample weight needed to produce estimates for the civilian, non-institutionalized population of the US in the designated year. The weight on each file (WTADJyy) is derived from the 2002 NHEA-aligned MEPS weight. It is adjusted to reflect changes in the non-institutionalized civilian population between 2002 and the designated end year in a three step process. First, weights are adjusted to reflect population growth by age, race and sex based on historical and projected population estimates issued by the U.S. Census Bureau13 . Second, the weights are adjusted for changes in mortality rates by race and sex. The third and final weight adjustment is for fertility rates by race. The fertility and mortality adjustments are based on historical statistics and projected estimates for U.S. births and deaths by year14, 15 .

3.1 Population Estimates and Projections

Individuals in the 2002 NHEA-aligned MEPS sample are placed into cells based on age, race and sex categories to match definitions in the U.S. Census Bureau’s population projections. The categories are broken down as follows: (i) Age: each year from 0 to 99 and 100+, (ii) Sex: Male, Female (iii) Race: a) Hispanic, b) White alone, Non-Hispanic, c) Black alone, Non-Hispanic, d) Asian alone, Non-Hispanic, e) American Indian and Alaskan Native Alone, Non-Hispanic, f) Native Hawaiian and Other Pacific Islander Alone, Non-Hispanic and, g) Two or More Races, Non-Hispanic. Population growth rates are calculated for each age, race and sex cell from 2002 to the designated end year using the Census projected population totals by age, race and sex. These growth rates are then applied to the person-level weights in the 2002 NHEA-aligned MEPS file by age, race and sex.

3.2 Fertility Adjustments

The second step in the re-weighting process involves fertility adjustments. These adjustments are made to population weights for two primary reasons: 1) fertility rates are expected to change significantly over the projected period for certain sectors of the population and 2) fertility events are associated with relatively high medical expenditures. It should be noted that the fertility adjustments are somewhat different from the population adjustments.

For fertility adjustments women in the 2002 NHEA-Aligned MEPS were placed into the following race cells: a) White Alone, Non-Hispanic b) Black Alone, Non-Hispanic c) White Alone/Black Alone, Hispanic, d) Asian Alone, e) American Indian and Alaskan Native Alone, f) Native Hawaiian and Other Pacific Islander Alone and g) Two or More Races. Note that only categories a-c account for ethnicity. Census estimates are unavailable for categories d-g broken down by ethnicity.

The MEPS sample does not contain enough births to support fertility cells based on age and race and require the assumption that fertility changes over time are constant for women of all ages in each race category. This prohibits researchers from investigating fertility for particular age groups, such as teens or older women, who may be expected to have greater fluctuation in fertility over time relative to other women within their same race category.

Fertility growth rates are calculated for each race cell from 2002 to the designated end year using historical statistics and projected estimates by race from Census16 . These race specific growth rates are then applied to the person-level weight for each woman who gave birth in the 2002 NHEA-Aligned MEPS sample17 . In order to maintain a constant total population by race, a final adjustment is made to the person-level weights for those women who did not give birth in the 2002 NHEA-Aligned MEPS sample. The size of this adjustment depends on how many women in the race cell gave birth and the size of the growth adjustment applied to the women who gave birth.

3.3 Mortality Adjustments

The third and final step in the re-weighting process involves mortality adjustments. These adjustments are identical in practice to the fertility adjustments, except that they are applied to both men and women. The adjustments are made for two primary reasons: 1) mortality rates are expected to change significantly over the projected period for certain sectors of the population and 2) mortality events are associated with relatively high medical expenditures

Individuals in the 2002 NHEA-Aligned MEPS were placed into the following sex, race cells: (i) male, female (ii) a) White Alone, Non-Hispanic b) Black Alone, Non-Hispanic c) White Alone/Black Alone, Hispanic, d) Asian Alone, e) American Indian and Alaskan Native Alone, f) Native Hawaiian and Other Pacific Islander Alone and g) Two or More Races. As with fertility, only categories a-c account for ethnicity. Census estimates are unavailable for categories d-g broken down by ethnicity.

The sample does not contain enough deaths to support mortality cells based on age, race and sex, and requires the assumption that mortality changes over time are constant for individuals of all ages in each race and sex category. This constraint prohibits researchers from investigating mortality for particular age groups, such as infants or the elderly, who may be expected to have greater changes in mortality over time relative to other individuals within their same race category.

Mortality growth rates are calculated for each race cell from 2002 to the designated end year using historical statistics and projected estimates by race and sex from Census18 . These race and sex specific growth rates are then applied to the person-level weight for each individual who died in the 2002 NHEA-Aligned MEPS sample. In order to maintain a constant total population by race and sex, a final adjustment is made to the person-level weights for those individuals who did not die in the 2002 NHEA-Aligned MEPS sample. The size of this adjustment depends on how many individuals die in a given race, sex cell and the size of the growth adjustment applied to those who die.

4. Calibration to National Health Accounts

The last step of the aging process is a calibration of each re-weighted NHEA-aligned MEPS file to the National Health Expenditure Accounts for the same year by type of service and source of payment. This process ensures that growth rates in NHEA-aligned MEPS expenditures by type of service and source of payment between 2002 and a designated end year will be in accordance with growth rates predicted by the NHEA.

In order to calibrate NHEA-aligned MEPS to the NHEA, it is first necessary to obtain expenditures for nursing home and assisted living for 2002, because MEPS does not include these expenditures in the survey. These totals were obtained from Actuarial Research Corporation (ARC)19 by type of service and source of payment for 2002. For years beyond 2002, the ARC totals are aged by the growth rates projected by CMS in the National Health Expenditure Accounts for nursing home expenditures by type of service and source of payment.

The calibration procedure is as follows for each designated end year: (i) Expenditures from the re-weighted NHEA-aligned MEPS file are totaled by the seven type of service categories and the nine source of payment categories for a total of 63 TOS-SOP cells for a given end year. (ii) Nursing home and assisted living expenditures are added to the 2002 NHEA-aligned MEPS expenditures by type of service and source of payment. (iii) "Aged" nursing home and assisted living expenditures for the designated end year are added to the re-weighted NHEA-aligned MEPS file by type of service and source of payment. (iv) Growth rates from 2002 to the designated end year are calculated for each of the TOS-SOP categories for the NHEA-aligned MEPS re-weighted data. (v) Growth rates are calculated for the corresponding TOS-SOP categories for the NHEA using historical and projected data based on 2005 estimates from CMS. (vi) Using the NHEA and NHEA-aligned MEPS growth rates, a calibration factor20 is created for each TOS-SOP category for the designated end year. (vii) Expenditures in the re-weighted NHEA-aligned MEPS file are multiplied by TOS-SOP specific calibration factors.

5. Additional Data Details and Caveats

It is important to emphasize that there are several sources of error associated with the NHEA-Aligned MEPS projected expenditures and with any estimates produced using these data. So, while the NHEA-Aligned MEPS projected expenditure file does not include variance measures and the summary tables do not include standard errors, these data are surrounded by several sources of uncertainty. Sources of error include sampling error in the MEPS and NHIS files, variation resulting from the data collection and projection techniques used by Census Bureau and CMS in creation of the Census and NHEA files, uncertainty in ARC estimates of the nursing home and assisted living population, variation resulting from the alignment of the 2002 MEPS and 2002 NHEA files and, finally, variation from the procedures used to age the MEPS population and calibrate aged expenditures to NHEA growth.

Researchers should note that the current release of projected MEPS expenditures differs from those in previous years. Projected expenditures from the 1996 MEPS were calculated directly from the 1996 MEPS-HC Full Year Consolidated Public Use File (HC-012). While research and statistics were available to explain and reconcile differences found in the 1996 MEPS and 1996 NHEA totals21 , none of these adjustments were applied to the MEPS file used in the expenditure projections. By using the 2002 NHEA-aligned MEPS instead of the 2002 MEPS Full Year Consolidated File (HC-070), the current release has been improved and takes into account differences found between the 2002 MEPS and NHEA totals. However, it is important to recognize that the projected totals are no longer directly comparable to the MEPS-HC Full Year Consolidated Public Use Files.

6. File Variables

The output files contain person level data with (i) personal medical expenditures classified by eight (including a total) type of service and ten (including a total) source of payment categories, (ii) a person weight that enables national-level estimates, (iii) individual level demographic characteristics used in the re-weighting procedure and iv) insurance and poverty status variables, not controlled for in the aging process. 6.2 Control Variables:

AGESTUB, RACE7 and SEX were used as part of the population aging process to create new person weights based on growth in the interim Census population projections by age, sex, race and Hispanic origin.

6.3 Variable Definitions and Codebook

Variable Definitions
Key expenditure and control variables
DUPERIDX Unique person identification number for each individual on the NHEA-aligned MEPS file and on each Projected NHEA-aligned MEPS file. This variable cannot be used to link individuals across years and cannot be used to link to other MEPS data sets. Person identification numbers that would enable direct linkage of the projected MEPS data to previously released un-aged MEPS files were purposely omitted to prohibit users from making projections for population groups that were not statistically controlled for in the aging process. Furthermore, these expenditures represent a single cross-section of individuals aged to different points into the future. The omission of linkable ID’s discourages users from incorrectly interpreting these expenditures as a panel of expenditures for a given individual over time .
PERWTP 2002 person weight from the NHEA-aligned MEPS file. This variable appears only on the 2002 Base file, it does not appear on any of the projected year files.
WTADJyy 2002 person weight from NHEA-aligned MEPS file, adjusted to end year YY for population, mortality and fertility growth. These weights allow researchers to get national level population estimates. Researchers should be careful when sub-setting by control variables. Un-weighted cell sizes must be greater than 100 individuals to obtain nationally representative estimates. Cell sizes can get very small when using variables such as age (especially when using single year categories). This variable does not appear on the 2002 Base file, only on the projected year files.
AGESTUB Age of individual as of Dec. 31, 19yy. Note: A similar age variable used for population re-weighting was top-coded at 100. The variable released on this file is more restrictive for confidentiality purposes.

TOSSOPyy

Medical expenditure variables by type of service and source of payment. The variable contains actual expenditure amount in dollars. Each year there are 80 variables covered by the type of service and source of payment categories. See section 2 for detailed information on the naming convention of these variables. Section 2 will also provide detailed information on the mathematical relationship between these variables and how the variables can be summed to create expenditure totals for major type of service categories.

Additional variables, not statistically controlled for in aging

The following variables are created for the convenience of end users of the projected MEPS expenditure data. These variables were NOT statistically controlled for when aging the data. Researchers should be cautioned that any estimates using the projected data cross-classified by poverty status or insurance status are based on the assumption that 2002 poverty and health insurance conditions remain unchanged in each future year.

INSCOV_A

Insurance status for individuals under age 65. (AGESTUB < 65) This variable is an annual measure, measuring whether the individual had insurance at any point during the year. Because individuals can have coverage from multiple health insurance sources, this variable is built hierarchically. All individuals ages 65 and over, regardless of insurance status, are placed in category 5; INSCOV_B will measure insurance status for these individuals. Note: Employment related private insurance in category 1 does not include self employed persons with firm size of 1 who purchase health insurance for themselves. These individuals would be assigned, instead, to category 2.

Definition:
1 = Any Private, employment related or TRICARE insurance during the year
2 = Any Private, not employment related insurance during the year
3 = Any Public (Medicaid, SCHIP or Medicare) insurance during the year
4 = Uninsured for entire year

INSCOV_B

Insurance/Medicaid/Medicare status for individuals age 65 and over. (AGESTUB >= 65) This variable is an annual measure, measuring whether the individual had insurance at any point during the year. Because individuals can have coverage from multiple health insurance sources, this variable is built hierarchically. Several categories of this variable are intended to measure whether individuals held multiple coverage sources at the same time. For example, category 3 includes an individual who was covered by both Medicare and a privately purchased policy in the month of February but does not include someone who was covered only by a privately purchased policy in January and covered only by Medicare in March. This second individual would fit in category 4. Note: Employer insurance in category 2 does not include persons who were self employed with firm size of 1 who had purchased health insurance for themselves. These individuals would be assigned, instead, to category 3.
