For each patient and for each event of service, we need information about services provided during a specific calendar year:
- Date(s) of service
- Diagnoses or conditions (DSM 5 codes, ICD-10 codes or descriptions)
- Services provided (CPT-4, DRG, revenue codes, HCPCS, or descriptions)
- Charges for each service provided
- Method of reimbursement (fee-for-service or capitated)
- Sources that paid for the service (if insurance, please specify Medicare, Medicaid, private, etc.)
- Amount paid by each source
- Adjustment activity
- All names, roles, specialties and contacting information for all doctors who may have billed separately for each date of service
