When the authorization form packet is ready, you will receive an email from noreplyMEPS@rti.org with your unique username to access the electronic portal. The username is a randomized set of numbers and is connected to your email account. You can use the login over the course of the project.

You will be prompted to change your password after you first log in. We recommended you change the password to something you will remember. Once logged in, please go to the Download Authorization Form Packets page.

To protect confidentiality, each authorization form packet will be encrypted with a different password. Your password for the packet is [FILL_EMAILGRP]. This password is also in lower case.

You will have 7 calendar days to download the authorization form packet from the electronic portal. The authorization form packet link will disappear after 7 days.
